Forward Health Group, a leader in value-based population health solutions, is highly rated and praised by providers in the “2015 Population Health Management Performance” report, which was released today by KLAS, a group that helps health care providers make informed technology decisions by reporting accurate, honest and impartial vendor performance data.
Forward Health Group received the highest preliminary rating scores for its solution PopulationManager® in the report: 90.1 out of 100, increasing its rating from last year by nearly 3 points. This shows Forward Health Group provides quality solutions for data aggregation, risk stratification, care coordination and patient outreach.
Echoing the sentiment of the high marks in the report, a Forward Health Group user noted, “The product is helping providers move to a population health mentality. We can show providers how they are doing with metrics, and we can show providers lists of patients who are not meeting certain metrics. We can give providers a place to start working so that they aren’t just working longer and harder; they can work smarter in order to meet patients’ needs… The product is being accepted well by providers, and it will hopefully help them provide better care for patients.”
Forward Health Group was recognized for provider satisfaction, relationship and outstanding service. A few findings from the report include:
The KLAS report examined 24 vendors in the Population Health Management (PHM) market to provide insight into the types of solutions that are available today, focusing closely on validating vendor functionalities and capabilities through conducting 213 interviews. Forward Health Group shined in this report by helping ambulatory practices, clinics and medical centers obtain the goal of improving population health and meeting the objectives of quality initiatives and pay-for-performance program through providing trustworthy data, engaging doctors and leveraging and optimizing current EMRs.
“PopulationManager enables us to better serve our 11,000 Medicaid patients by unlocking our data and presenting it in an actionable way,” states Beth Wrobel, CEO at HealthLinc, client of Forward Health Group and participant in The Guideline Advantage™. “In a few short months, we have seen an uptick in screening rates among several key patient populations, including those diagnosed with diabetes, cancer and hypertension. Forward Health Group knows and delivers data we can trust.”
“As the payment model shifts to value-based care, the way organizations look at patient information also changes. With minimal capital outlay, we work hard to empower docs with the data they need that truly adds value. We believe our KLAS performance scores affirm and confirm what we’ve accomplished so far,” says Michael Barbouche, Founder and CEO of Forward Health Group. “We’re thrilled KLAS recognizes how our solutions provide more visibility, information and insight into the health and well-being of patients, enabling doctors to better serve.”
